As a rule, commissions should be issued as required by sections 75 and 76 of the Code and in Form No. 7 or 8 of Appendix H of Schedule I, though in special cases commissions for the examination of witnesses not named, and whose names cannot be ascertained by the parties applying for such commissions, may be allowed.
