Each file of every record shall have prefixed to it a title page of the following description :—

1. Name of the Court.

2. Case Number.

3. Names of parties.

4. Date of decision.

5. Numbers of Exhibits included in the file.

6. Category of the file (as 'A', 'B', 'C' and 'D'.)
