(1) All documents tendered in evidence shall be accompanied by a list in the form given as No. 5 in Appendix H of the first schedule of the Code of Civil Procedure. (2) If any document is written in pencil or is torn, moth-eaten or damaged in any other way, a clear note to that effect must be made in the list and any such document shall be brought to the notice of the Judge by the clerk whose duty it is to receive it.
